Section 504

The 504 plan refers to Section 504 of the Rehabilitation Act and the Americans with Disabilities Act, which specifies that no one with a disability can be excluded from participating in federally funded programs or activities, including elementary, secondary or postsecondary schooling.  

When there is concern that a student exhibits a mental or physical impairment that substantially limits one or more major life activities, they may be eligible for a 504 plan.  A referral may be made to the school 504 coordinator, or directly to the District 504 Coordinator: Dana Cotter, Supervisor of Special Services (869 7750 x7313).  

A meeting and review of concerns will be conducted to gather information from a variety of sources on how the disability is substantially impacting a major life function.  Evaluation procedures may include review of records, assessment information, input from team members, observations, and medical documentation. The team will be comprised of members who are knowledgable about the student, understand the meaning of the evaluation data and are familiar with placement options.   The purpose of the meeting is to examine the information regarding the individual, and determine if reasonable accommodations are necessary, and subsequently what those accommodations will be if the individual is considered eligible for a 504 plan.
